Annual diabetes review. HbA1c: 58 mmol/mol β€” above target (aim <53). Weight: 94kg, BMI: 30.2. BP: 144/88 mmHg. Continues on Metformin 1g twice daily and Lisinopril 10mg. Increase Metformin counselled; patient to trial dietary changes for 8 weeks before dose adjustment. Referral to practice dietitian made. Foot check: no ulceration, sensation intact. Eye screening referral sent. Repeat HbA1c and U&E in 3 months.

Patient called with 4-day history of lower back pain following heavy lifting at work. No red flag symptoms (no bladder/bowel changes, no leg weakness, no fever). Advised rest, regular paracetamol and ibuprofen (with food). Safety-net advice given. Referral for physiotherapy considered if not improved in 6 weeks. Fit note issued for 1 week (unable to perform manual handling).

Dr F. Nguyen – Diabetology, St Mary's Hospital
Outpatient review for Type 2 diabetes management. HbA1c trend improving from 67 to 61 mmol/mol over 18 months. Current regimen of Metformin adequate at present. SGLT2 inhibitor (Empagliflozin) discussed as future option if glycaemic targets remain unmet; to be initiated in primary care if indicated. Renal function satisfactory. No evidence of diabetic nephropathy. Discharged back to GP care with 6-monthly HbA1c monitoring.

Dear James,

Your latest HbA1c result has come back at 61 mmol/mol. While this is a small improvement from your previous reading, it remains above our target of 53 mmol/mol for diabetes management.

I would like to discuss whether we should consider adjusting your Metformin dose or adding a second medication. Please book an appointment at your earliest convenience.

That's great to hear, James. In the meantime, try to reduce refined carbohydrates and sugary drinks. Regular walking after meals (10–15 minutes) can also help lower post-meal blood sugar. We'll review everything at your appointment.
